Lip Filler Aftercare: Exactly What to Do in the First 48 Hours

Why Aftercare Matters

Lip filler works. But how your lips look at the 2-week mark depends partly on what you do in the 48 hours after your injection.

The right aftercare reduces swelling, minimizes bruising, protects the placement of the filler, and lowers the (already small) chance of complications. Wrong aftercare doesn't ruin your results - but it can make the first week uncomfortable and the bruising worse.

Here's exactly what I tell my patients.

The First 24 Hours - What NOT to Do

❌ Don't exercise, do hot yoga, or sweat hard

Raised blood pressure and increased circulation make swelling and bruising worse. Skip the gym, the run, the hot yoga, and the sauna for 24 hours.

❌ Don't lie flat for the first 4 hours

Gravity matters. Keeping your head above your heart helps swelling drain. Sit upright or recline rather than fully flat. When you sleep that night, use an extra pillow.

❌ Don't drink alcohol

Alcohol dilates blood vessels and thins the blood. Both increase bruising. One day off.

❌ Don't kiss, use straws, or use a sports mouthguard

Pressure on your lips during the first 24 hours can migrate the product. Lips are soft tissue holding a gel - the gel moves if you push it. Avoid anything that pushes on your lips repeatedly.

❌ Don't take ibuprofen (Advil), aspirin, or other NSAIDs

Unless medically necessary. These are blood thinners that increase bruising risk. Tylenol (acetaminophen) is fine for any discomfort.

❌ Don't take fish oil, vitamin E, turmeric, or ginkgo supplements

All mildly blood-thinning. Resume after 48 hours.

❌ Don't apply heat

Heat increases swelling. No hot compresses. No hot showers on the lips. Keep water warm, not hot.

❌ Don't aggressively touch, rub, or massage your lips

Gentle cleansing only. Resist the urge to keep feeling them - I promise they're fine.

The First 24 Hours - What TO Do

✅ Apply a cold compress (carefully)

A cold compress reduces swelling. But a few rules:

  • Never apply ice directly to your lips - you can cause a cold burn on already-delicate tissue.
  • Wrap ice in a thin cloth or use a gel pack.
  • 5 minutes on, 10 minutes off. Don't leave it on continuously.
  • Use in the first 24 hours only. After that, heat/cold doesn't help.

✅ Sleep slightly elevated

One extra pillow. Gravity will continue to reduce swelling overnight.

✅ Stay hydrated

Water doesn't dissolve filler (a common myth), but hydration helps your tissue recover. Drink normally.

✅ Eat normally - carefully

You can eat anything. Use smaller bites. Avoid anything that requires wide mouth-opening (burgers, large apples) for 24 hours - not because it'll affect the filler, but because your lips are tender and wide opening feels uncomfortable.

✅ Cleanse gently

Your lips can be cleaned normally. Skip any aggressive scrubs, exfoliants, or lip scrubs for 24 hours. A gentle cleanser twice a day is enough.

✅ Use arnica if you bruise

If you see bruising starting (any mild purple), over-the-counter arnica (topical or oral) can help speed resolution. No evidence it prevents bruising, but some evidence it helps existing bruises fade.

Hours 24-48

You can reintroduce most of your normal activities, but still avoid:

  • Hard workouts
  • Alcohol
  • Aggressive facials or exfoliation on the lips
  • Sleeping face-down

And continue:

  • Sleeping with head elevated
  • Gentle skincare only
  • Hydration

Peak swelling is typically at 24-48 hours. Your lips will look larger than the final result during this window. This is expected. Do not panic. It will resolve.

Days 3-7

  • Most swelling has resolved by day 3.
  • Any bruising starts fading from day 3.
  • You can resume normal exercise at day 3.
  • Firmness (palpable lumps) may persist - these usually soften over 2 weeks.
  • Photos? Wait for your 2-week follow-up.

What's Normal (Don't Panic)

  • Peak swelling at 24-48 hours, larger than final result
  • Minor bruising in 10-20% of patients (resolves in 3-7 days)
  • Small palpable lumps or firmness (soften over 2 weeks)
  • Mild tenderness for 3-5 days
  • Tiny pinpoint red marks at injection sites (fade in 24 hours)
  • Asymmetry in the first week as swelling resolves unevenly
  • Feeling that your lips feel "different" while you adjust

None of these are reasons to worry. Lip filler always looks different in the first week than the 2-week final result.

When to Call Me Immediately (Rare But Important)

Call 613-869-3269 right away if you experience:

  • 🚨 Skin that turns pale, blue, or grey-ish (especially if it's different from a normal bruise color)
  • 🚨 Severe pain disproportionate to the treatment (lip filler is usually mild discomfort, not sharp pain days later)
  • 🚨 Vision changes of any kind
  • 🚨 Severe headache or nausea
  • 🚨 Skin that looks white or is extremely cold to touch

These are rare but can indicate vascular occlusion - a complication where filler has inadvertently compressed or entered a blood vessel. It needs prompt treatment. As an MD, I can dissolve HA filler immediately with hyaluronidase if needed.

Regular bruising, swelling, tenderness, or a firm lump is not an emergency. But any of the above is - call me.

The 2-Week Follow-Up

At 2 weeks, we review your lips together. This is the right time for:

  • Assessing final shape and volume
  • Discussing whether any small adjustment is warranted
  • Taking "after" photos

Do not judge your filler before 2 weeks. I've had many patients come in at day 3 convinced something's wrong, and by day 14 they're thrilled. Trust the process.

Long-Term Lip Care

Your lips last longer and age better with simple ongoing care:

  • Daily SPF lip balm - UV damages lip tissue and fades filler faster
  • Hydration - chronic dehydration shows in lips first
  • No smoking - beyond the obvious, smoking affects lip appearance
  • Gentle exfoliation (resume at 2 weeks) - a soft toothbrush or sugar scrub once a week
